It sounds like doctors are still grappling to work out best practices (not surprising with a new disease) and darko_yolo already answered the more sensational idea that ventilators were killing people who would certainly have survived.

Before you put this on the doctors, remember that there are also a lot of people dying at home, because, you know, this pandemic kills, sometimes.
