In true capitalism doctors wouldn’t be bound to hospitals but could open small clinics where minor injuries like broken bones, stitches, burns and some minor surgeries can be treated. Then more hospital resources could be allocated to major medical needs and more medical staff can be hired to perform these. This would serve to make healthcare more accessible and affordable to everyone. All they would need is access to a building to practice in and the tools needed to do such medical work. In communism these would not be possible because nobody can own the means of production privately.

They still wouldn’t do it for nothing. They would trade their services for something of equal value. In other words, you would still have to work.

In that case they would have to find someone who needs what they have to sell and simultaneously has something they need, like a new tool or repair work. It relies too much on coincidence of needs, which is actually a rare occurrence. What are the odds that in a community of 100 I would find someone who had extra tomatoes and needs leather that I make?

But we solved that problem thousands of years ago by creating a common medium of exchange for every exchange that we make. It’s called “Money.”
