No, survival of the fittest does indeed mean the ability to survive new illnesses, diseases, pandemics, etc.

Prime example, ask anyone with Cystic Fibrosis (CF). CF carriers and those that have it have ancestral roots from northern Europe. Scientists have concluded that those that were CF carriers during the plague back in the day were better suited to survive the plague. High survival rate, higher chances of having kids and passing down your genes (survival of the fittest). The only problem with CF is when two carriers get together to procreate then each child has a 25% (1 in 4) chance of being symptomatic with CF and having a historically shortened lifespan. Luckily, due to our evolutionary advanced brains we've found better ways to diagnose and treat CF and are vastly increasing lifespans. Still working for a cure.
