Yes, it's my template. It's from Black Butler Book of Murder, and I wouldn't be surprised if half of the Black Butler templates on imgflip are mine, lol.
I've been using Shadoe online since 2008 when I made an avatar on SecondLife and wanted something that could work for my avatar whether it was male or female at the time, since the avatars in SL can be changed to pretty much anything you want at anytime. I was even a cheeseburger for a while.

I'm not a pro, I just go through stages of liking (almost obsessing) over different shows/book series/etc., and I'm on a Black Butler kick right now, so I've uploaded a bunch of templates from it. I'm just getting over phases for Hellsing and for Jeeves and Wooster, so I've uploaded several templates for them as well.
