The most reasonable answer I have is that it slowly does 00.1 Damage overtime, which wouldn't kill him, but slowly drain his 1 HP until it's completely 0. That would also damage his soul, but since in Genocide we never see Sans or Papyrus' soul break, we never know if they're actually dead(Besides the player level up)
