Well generally speaking, the opioid issue is largely focused on America because you allow massively addictive drugs to be given as painkillers. People get addicted and they either keep taking them or move onto harder drugs.
Overall though, Even here in Europe we have major drug and suicide issues. I suspect its because modern living and society is just too hard and toxic and people feel the need to escape or just outright fail and end up in the gutter.
I gave up all that rat-race, consumerist nonsense many years ago and I haven't been stressed, anxious, tired, fed up with life, financially struggling or over-worked since. --The idea of taking drugs or suicide is unfathomable in my lifestyle, but not in alot of others, and maybe thats the problem.
