Most psychiatrists guess symptoms; "Little Johnny couldn't focus in school, he has ADHD" when really it could be something else. I think Psychiatrists, like Gastroenterologists, should have scans of the brain. Amen Clinics does this, an explains so in his book "Change Your Brain Change Your Life". But its really expensive, due to liability costs. Because ADHD doesn't only mean inability to focus, it also means you struggle with executive function, learning disabilities, and impulse control.
