That would be reallyitsjohn ;) He made a lot of these that are really good, in fact there is one just like this one....

It would have been Clinkster, his templates are great :)

The template was most likely made by clinkster, not many know it but that guy makes a ton of templates and submits them for all to use. He even has his own e-book out that he will let people download for free ;)
