Okay, so let's break it down (spoiler alert btw): So the succubus that Guts was f**king in the very beginning, that was the same succubus that killed one of his comrades, Corkus (although Guts didn't watch that happen, so he likely did it to get her to reveal her apostle form so he could kill her). Moving on to the bar, the thugs are from this castle guard that has control over the mayor of a town who keeps sending them women and children as food to keep the Lord of that castle (Koka Castle), who is also an apostle, from attacking them. Guts kills the thugs because why not and the Lord gets pissed and attacks the town while Guts is in the dungeon, but Guts escapes and the Lord goes into apostle mode, and Guts kills him and Puck does a title drop.
