The blind guy next to it is reading a newspaper, I know he's blind because of the stick next to him, it's commonly used by blind people to help figure out what's in front of them
(I would've mentioned what's on his plate but I can't tell what it is, however it could be human fingers because of the handprint and the pie)
The lady at the counter is cutting a bloody pie, which would explain the hand print
In front of that the lady pulling her child has a tentacle arm, and if you look near her feet you can see more tentacles, and if you look to the left of the child there's even more
And finally, the first thing a lot of us noticed, the skull in the coffee
