No-one knows how Sans or Papyrus were actually created.
Sometimes they're his kids, sometimes friends or sometimes brothers.
No-one can actually say for sure about it, but from my perspective, he created Sans as an assistant, but soon became like a father over him and created Pap to keep him happy while he does work
