I see what you're missing. The Attorney is trying to catch the witness on not checking to see if the patient was still alive. The witness says the patient's brain is sitting in a jar (the patient is brainless). The attorney asks if the patient could've still been alive. The witness says "yes, its possible that [the patient] could've been alive and practicing law", saying that the attorney is brainless, comparing the patient to him.
