People are in all 50 States too. But the infections are concentrated...you know...like the map shows, which is the point of the meme. Density correlates with the infection rate. Some (emphasis on the word some) have claimed that this virus will target Trump supporters more than anyone. The chart makes a point that they shouldn't be so ignorant as to forget how infectious diseases work. Population centers are the first to fall in any apocalyptic scenario. Those of us who live our lives "social distancing" mosts of the time anyway, aren't at the same risk, but lower.
