If it features and goes back to submitted, that could very likely be from people downvoting or flagging it. If it never features, that would mean it was the mods holding it back.
Unfortunately you can't monitor your memes all the time, so sometimes you check back and it's still in submitted, and you don't know if it had a brief period where it featured or not - so it's really hard to tell.
