I think they only get involved if it's an original image. Memes on popular templates mostly feature automatically in certain time-slots (I'm not sure about what all the time-slots are). There might be certain "no no" words you could use on a popular image that would bump it back to get approved by a mod's eyes, but other than that they seem to just go through on their own.
