Tbh, I hate that. What am I supposed to do? Where am I supposed to look? How am I supposed to react? It's a bunch of total strangers shouting at me, while clapping.

A simple "Happy birthday!" will suffice. If you *must* go one step further than that, a free cupcake is plenty sufficient.
