That's strange, I've yet to see it fail! (to be clear I'm talking about Adblock Plus, I can't comment on the performance of the independent extension Adblock) I suggest you Google possible solutions because it should work pretty much 100% of the time.

Yes, I hear Adblock has more trouble with video ads, so that could be it. Try ABP ;)
