Rules: Unlike the other games where I’d play to see who wins, you name an attack and I’ll roll three dice twice, once for OMORI and once for YOU.
If your roll is higher than OMORI, you win and receive a shoutout meme and a follow. If you lose, OMORI pushes you down the stairs

Somehow, Mitsuri manages to use a gun and shoots him directly between the eyes.

Sukuna and OMORI trade blows, Sukuna landing many but OMORI refusing to succumb.
At last they strike one last time, and both end up dying.
While I don’t know how Sukuna works, I am pretty sure OMORI would just go back to WHITE SPACE.
For plot relevance or whatever I’ll just say Sukuna survives with anime plot armor
