A fuel cutoff caused the deadliest aviation disaster in a decade, according to a preliminary report. The Boeing 787 Dreamliner, flying from Ahmedabad to London on June 12, crashed and killed 260 people, leaving just one surviving passenger. A report by India's Aircraft Accident Investigation Bureau
states that the fuel control switches were off.
As the plane lost altitude the co-pilot asked the pilot
why he turned off the fuel; the pilot said he didn't.
They quickly turn the switches back on, but it was too late.
