I mean, she was first seen in the episode 'The Holy War' with a bleeding mouth, eh? It makes sense.

But in every other thumbnail she is too, even the peaceful ones

I'm assuming that Matthew considers it a very notable part of her character design in the early years of the series. Maybe it represents Kopis' toughness as one of Xiphos' wives?
