Just looked it up, apparently it is the smell, not just from your breath but across, over and throughout your entire body as well. It overwhelms a vampire's heightened senses. Since 60% of what you taste is what you smell, the garlic smell would logically make your blood taste like garlic too. In high enough amounts, the sheer sensory overload could cause the vampire's system to shut down to avoid damage, effectively killing it.
