Israel has the most to lose here so I don’t see them as the culprits

Hamas could’ve misfired a rocket or they could’ve “accidentally” hit their own hospital to drive up support against Israel

Iran could’ve targeted the hospital via their insiders with Hamas to hurt Israel’s image or to stop Biden’s negotiations
