One step? Piking. It makes crucifixion look like hospice care. A skilled torturer could miss vital organs, leaving the victim to die from infection due to a ruptured bowel, often days later. In many cases the pike would push around the heart and lungs and exit through the victim's mouth. Certainly there are worse ways to die, but as far as 1-step punishments go...
