This mushroom is called the Death Cap (Amanita phalloides) it is one of the most toxic mushrooms in the world. Upon consuming it, you will die a slow and painful death within 72 hours. The symptoms upon consuming this mushroom include abdominal pains, nausea, vomiting, diarrhoea, Liver/Kidney failure, and of course death. There is no antidote.

I’m not sure how common it is. There are other species of Amanita like this which are equally as poisonous.
