So after he got six of the many infinity stones in the multiverse he wanted to test his new power by fighting The Puppet Master. This was a bad idea because the puppet master had mutilated Dr. Lavikich so much that he was pretty much undead. Since the puppet master had ripped off so many of Dr. Lavikich's body parts (Including his vocal chords) he had to make new, mechanical body parts, that included a voicebox. So that's why his face is like that and why he has mechanical parts
