“Compliance” with all medical requirements for transplant (testing, vaccines, drug and alcohol testing dental care (no open cavities for infections)) is standard across US transplant centres. Non compliance with basic medical protocols is associated with poorer outcomes post transplant. I’d be more worried about candidates being refused transplant for not having access to $10K for first 3-6 months of meds.
