You change your pronouns, and you start presenting how your gender relfects- so if you were ftm then you would probably get a chest binder, and start presenting more masculine, and start testosterone

it's called transgender because you are TRANSitioning to a different GENDER
most people start the transition by changing their pronouns, although not everyone who changes their pronouns are trans
then those who are trans, when they have the money, they get the surgery to do the FULL transition to the other gender

people change pronouns cause they feel more comfortable doing so
look gender dysphoria is a bitch ok
people change their pronouns to help ease the anxiety caused by it
