The first time I saw it was from a mini series years ago, where it was them doing activities, and all dying from them. One drove a racecar, then died from the amount of exhaust. One cut their lip sealing an envelope, and blood legitimately filled the screen and they drowned, one was hulahooping using a lot of hoops, and the dad kept giving them more, not paying attention, and she cut herself into tiny pieces, and the dad continued throwing hoops, not knowing they're dead
