We do want to handle this better. The issue is that people who swear in the image are waaaaay more likely to be doing something else that is bad other than simply swearing (such as attacking other users or other rule violations), so we need to add extra scrutiny while approving those images and keep them NSFW by default. Mods can still remove the NSFW tag while they're approving it, if it's tame.
