Click on their account: you'll be able to spot em in five minutes. They got few images, a bunch of comments & somehow know everything that happened on imgflip for the last year even tho they've been on here 10 days.

What about the people that make alt accounts in advance and use them like normal accounts to keep people from suspecting them?

You can easily spot those too.
Example, a certain alt account on here who harrasses a bunch of people, [Redacted:]
●Click on the account
●He has about 15 lifetime images and only 15,ooo points in 7 months. Obviously not someone's main account.
●All his comments are attacks, obvious trolling, and cluster[redacted] people in the comments section. Clearly an account made simply for illicit purposes.
●See who he simps for: you found your culprit. Troll accounts are almost always the 'SECRET' attack account of an established user who doesn't want to get their main account deleted.
As for those people that actually run a bunch of totally legitimate accounts with seperate images and personas: they are the exception that proves the rule.
Example: if you decided some day to alt, only your friends would know it was you, because you're not on here harassing people 24/7. The people that bend the rules for whatever reason but don't break TOS by making alts simply to troll aren't an issue on this stream.

The problem with having alternate accounts is that most people assume anyone with alts is bad.

Right, cause there's only a few reasons to alt.
RP (people like to have fun, not against TOS.)
Upvoting your own stuff (silly, but not against TOS.)
Trolling without consequences (making alts for the sake of harrassment is against TOS.)
Impersonating people (definitely agaisnt TOS.)
But my point was: the worst of the worst (the alts we have on here who attack people) are easy to spot.

And the sheer number of alts on Imgflip has caused people all over the site to accuse each other of being alts.
Someone even accused 𝘮𝘦 of being an alt once.

That's cause some top users once found out that half the accounts on that hellhole 'Politics' stream weren't unique users. So people said: half the people screaming at me in the comments are fake?
This led to people thinking everyone is an alt and everyone is breaking TOS. (You can't make an alt for the purpose of harrassment.)
But even if you did make another account, some day, you wouldn't use it to troll, so it wouldn't be a problem. See what I mean?
