My theory is that the ghosts are whatever gender their host is.
Mettaton's robot bodies are male, so Mettaton is male while those are his host.
The Dummies are genderless, so Ruins Dummy and Mad Dummy remain genderless while those are their hosts.
The Mew Mew doll is female, so Mad Dummy is female while that is her host.
