When you say "plain", an experienced McDonalds worker already knows that it's only with cheese unless it never came with it to begin with. You don't need to say "no ketchup, no mustard, no onions, and no pickles" right after saying "plain" because they are one in the same... besides, it makes you look retarded, and not even in the endearing "I can't help it" kind of way...
