Question 1: See there's also a door to the right?. Very likely the ladies restroom (since the one on the left is the gentlemen restroom), of course, the picture didn't show enough to confirm that one.
Question 2: That one is a more tricky one. I cannot see the entire room, but having it too close to the "ladies" or "gentlemen" sign may not be the best of ideas... Personally I wonder... This is clearly a sub-corridor apart from the restaurant itself, so I wonder if it would be best if the sign was hung at the entrance of that corridor... But of course, I don't know that location... And yes I am sure of that, all McDonald's restaurants I've visited so far had their signs in Dutch except for one where they were in French... I've only spent two weeks on ground where English was the official language (which happened to be Ireland)...
