I don't get it.

This is a Mario-Kart meme. And the Blue Shell gets to the player in first place and hits them. AND that makes people stressed. So, there's the joke

I get that, but it's about the image of the Blue Shell, I'm most likely wrong but the image makes it look like the person in first place is the one in possession of the shell.

There is an arrow on the bottom of the bubble, so it's heading toward them. If the guy HAD a Blue Shell, it'd be on the top left corner
