EBT (Used to be place) Complaint Template

Apparently "Shameka" is the reason why we can't get our EBT *reason:due to misinformation and we're waiting for the 'next card' based on what mom stated *we also don't live there anymore* and at the same time, were currently waiting for a new one... despite the fact it was already at that location, but misinformation was causing us to get a new one.
