I prefer to do both. I can't help but give it mentally with my face so I figure adding the words will help make things clearer to exceedingly stupid people who need more emphasis because they're so stupid that the "you're a retard" glare doesn't sink in. I do it to everyone because life has taught me to assume that virtually everyone is an idiot.
