So a normal Pearl is made when an irritant gets inside a mollusk. The mollusk then coats it in a hard material.
There's these mobs in the end called the shulker, they are basically mollusks of the end, and this is why some ender pearls have ender mites in them, they are irritants. Ender pearls come from shulkers, not enderman testicles
