I'm a bit late to this, but i think that "staying alive" is the Final Problem. Both Sherlock and Moriarty are so far beyond "ordinary people" that their problem is finding a meaningful reason to "stay alive." Moriarty especially feels like there is no reason to stay alive because he has beaten Holmes by destroying his reputation (or so he believes)
