... That could lead to even more mod abuse... Mods could flag legitimate moderations and say that they're not...

That would be quite solvable: If the flagged moderation links to the disapproved image or shows the deleted comment and also needs an explanation like an image flag needs one, the site mods could judge whether the moderation is legit or not and whether a mod removal or even a ban is needed or not. (Sry for the essay)
