I'm pretty sure it's based on the name of the stream, the description of the stream, and what is posted in the stream.

Yeah the description does say NSFW is allowed even though there’s never really NSFW content there, the only NSFW content that’s ever there is when Turkey posts p*rn

I think the logical explanation is how many nsfw images are posted in a stream.

Maybe most of the images in freestream are Marked "NSFW"
