Hey guys, Peter Griffin here to explain the joke. The person who I assume is the mother says, "Son, you need a girlfriend," to which, the person I assume is the son in question, retorts, "Why", in incredible confusion and disbelief. If you look at this picture for more than 2 seconds, you would notice that the son has a slightly larger right arm than his left. This little "easter egg" is very essential to the joke. If you didn't know, to get an arm that buff, you would have to have daily exercises. Now what this image is suggesting is that the son has been using his right hand to participate in the activity that is usually called masturbation. Now, as you realize this, the mother's statement makes much more sense (or it doesn't, maybe you just aren't as intellectual as I am). She's suggesting that the son get a girlfriend so that he would stop being such a dick-twiddler and have actual sex. It's clear that his zerkin' off is a great annoyance to the mother, as seen by her expression, and she clearly wants him to stop.
