Happens to me a lot

Sometimes text in an image or a tag can automatically turn a creation NSFW. Also sometimes (not saying this happened) users will inadvertently check the NSFW box during the title and tag editing process.

And of course, a Stream Owner/Moderator can update the NSFW flag during the approval queue process or later.

I have to do this in anime a f**kton. Like, the meme has sexual content (Not enough to break the ToS, like pasta) is appearently SFW
