In episode 16, it's shown that Zack and Ray jump out the window together...which couldn't have happened. Zack is shown with his scythe that BROKE inside the building which burst into flames and collapsed. He's also shown with his normal outfit and bandages, which most likely would've been exchanged for a prisoners outfit. I'm not saying Zack couldn't have escaped, but with his scythe (that broke) and his normal outfit + bandages is impossible. What most likely happened was something caused Rachel's window to shatter, there was a scream outside, when the window shattered, Zack was never there...it was probably a hallucination, but thinking Zack was there, Rachel jumped out and died. EVEN IF Zack was there, how would he have gotten to her window? There were no balconies, it was up off the ground...
