A Fast One is a term of art for a con man. It can be used in several ways to describe successfully deceiving people with a gimmick , or through other forms of manipulation. In this instance it was used to make people believe he would be doing something extraordinary that would benefit them, but it was blank paper and there was never a Health Plan to reveal.
