If you're a Moderator of a stream and you want to remove yourself, go to the stream homepage, #1 click the drop-down on the upper right, #2 the settings button will appear, click that.
On the settings page, all the Owners and Moderators will be listed. Find your username, there is a red "Remove" button. Click that and there should be an "Are you sure?" Pop-up to confirm.
