Good question... I'm just glad it's a drawing and not a photograph!.... It appears to be either a 'historical' drawing, or meant to look that way... If it's real, it's probably depicting some sort of 'medical treatment'(?), or maybe it was some sort of satire in its day?.... I just stumbled across it, searching for meme pictures... In any case, it fits 'Mueller' to a tee!
