Depends on how long you've been training. When it happened to me I was able to defend myself for about 3 minutes. Most of it's muscle memory and you can tell a great deal by weight shifts and changed stances and figure out what you're opponent wants to do. Offense is extremely limited because you can't tell what they may be blocking your attempt with (are they literally using their head to keep you from being able to grab ahold of their opposite arm, etc). If I would played defense above my waist and f**ked with his legs with my legs, I probably could have got out. This is the one time I didn't keep calm and relaxed in a bad position, because I'd never been in that position before. Even my instructor didn't enjoy me f**king with his legs with my legs last night. LOL, he refuses to tap out unless you lock something in 100%, even then he'll go hulk on your ass and use power to avoid tapping. LOL, he put me in a pretty defenseless position last night and play slapped me in the face. As soon as I got free I returned the favor, knowing it would be a mistake and that I'd be tapping out VERY quickly. It was worth it though...LMAO!
