The subtle thing in Obamacare was that it eliminated solo-practitioners. Doctors no long work for us, they work for the insurance companies. When an insurance company send out an Explanation of Benefits, they only pay the "contract" price with the provider of pharmacy and tehy get a "discount" when they pay the contracted price for paying within 90 days.
