The joke isn't about verbal punctuation, it's about people's lack of understanding and use of proper punctuation in writing. The meme wasn't spoken, it was written; you didn't hear the meme, you read it. Punctuation matters.

Since this is the second time someone has asked this, I'll answer: You pause. It's not a long pause, as one would expect with a period, but a shorter pause that is still quite noticeable. A child yelling "I LOVE THE CANDY MAN" sounds a lot different from a dude-bro saying "I love the candy, man." There's a slight pause between "candy" and "man" in the second phrasing that distinguishes between a "candy man" and a man you're talking to about candy. Punctuation matters, always.
